
A pending claim is a new claim not yet recognized by the Bureau of Worker's Compensation (BWC). It is initiated by your employer. Once the Bureau has processed the claim, it will assign a claim number to you for the injury. Just because you have a claim number does not necessarily mean that your claim will be paid as worker's comp.
C84 Form generates temporary compensation which you are paid if your injury is severe enough to cause you to be off work. You will receive a form from the BWC if this applies to you. It is your responsibility to complete your portion of this form and send it to our office for our forms specialists to complete the physician's portion of the form. This form has an expiration date and needs to be updated. You must send us your C84 form at least 2 weeks prior to the expiration date of the current C84 for us to request a continuation. C9 Form is used to request authorization from the BWC for tests or certain types of treatment you may need. This form is completed by our staff and forwarded to your MCO or claims adjuster. If your claim is denied, you will be responsible for any costs incurred for tests or office visits. Therefore, we will be requesting information regarding your private health insurance.
C86 Motion is used to request an additional diagnosis be added to your claim.
If you have more than one worker's comp claim, the Bureau requires that you are only seen for one injury per visit.
